People who will NOT start off missions until they have personaly approved each and every build. Also people who kick others from mission/quest groups because the build won't work in thier very limited opinon.

If it's a difficult mission that requires some sort of build synergy there's nothing wrong with that.

People that have given themselves the King-of-Builds-title and think they are better than others and know everything there is to know about builds and synergies.

People adding me and kicking me shortly after because - gasp - it's not a bunny thumper or touch ranger build, or freaking out because they just realized they invited a ranger.

I don't mind pinging my build much. It's good to get some synergy, and weeding out the really obvious shit like wars with Meteor Shower or ranger secondaries with Charm/Comfort Animal is never the terrible oppressive issue they make it out to be. Besides, if someone's correction is crap, about three people will call them on it within five seconds.
New pet peeves: people who get all passive-aggressive while arguing. You know how do to the mission/bonus/quest, they don't, but when you try to guide or lead, they're all "i've been playing this game since beta and i've done this a million times but w/e"
I'm not trying to be pushy or rude, but if I know what to do and no one else takes the lead first, I'm going to step in.

Bosses that party whipe you in one or two attacks. On normal mode. I handle it of course , but give the casuals a break. Also, why do even the total newbies have to get trough Shards of Orr to reach Gadd's encampment? The place EATS casuals.
And i don't like that the game records how many times you died. (720 now) I don't want Anet to know it. They can blackmail me.

1. People who continually whisper you with WTB, spam, insults, whatever and then put themselves on Do Not Disturb, that's lower than low..
2. People who try to boss others around in PvE or PvP, but don't seem to know the tiniest bit about tactics!
3. Rage-quitters.. who are often terrible players, only seem to live to insult other people, often have crappy builds or builds they stole but don't know how to play and then quit when things go badly and then wonder why they always lose LOL
And finally a personal favorite of mine:
4. Warriors that go after my flashy birdy pet everytime in AB while I stand there scratching my head as I completely negate whatever damage they're doing with Heal as One...

People who criticize builds mainly.
Unless the build is obviously flawed (Hamstorm), shut your mouth and see what happens. 9 times out of 10, you are not the smartest person in the party.

WTS Iron Ignots! That unfortunate misspelling has been around since Ultima Online. It made me laugh make in the day, but now it's just getting tiresome.

-People that use Mending in RA. It makes my heart sink when I hear the sound.
-People that don't pluralize. E.g. "WTB 10 Monk Tome."
-People that spell 'warrior with an O. 'Worrior'.
-People that spell 'scythe' like 'schyte'. Look the same, sounds very different. "WTB a schyte!!!12".
-People that try to skim more gold off of an item when you're already selling it dirt cheap.
-People that never turn Caps Lock off.
-People that say, "WTB Ectos 4.5k, WTS Ectos 5k" all in the same line.
-People on the Local side of Vizunah Square who let Togo die despite your stringent efforts to explain that he's likely to be targetted by Afflicted.

When you get KO'd in dwraven boxing dungeon it gives you full health and energy for a second because it has to reset it (for the number of times youve been ko'd) I thought I had full energy and I minimized for a second, only to come back to my dead survivor

As a casual player, all I can say is smite monks and/or Battledepths. :P
Edited to stay on topic: eye-grabbing seller spam, especially when info like prices or specifics are tossed aside to make room for pointless punctuation, like "@@@@@~~~*~*~--->WTS GREENS<---~*~*~~~@@@@@"
Edit 2: I haven't done Shards of Orr since GW:EN's release, but it wasn't so bad except for the mobs camping the rez shrine. :P

On the flip side, people who think that their beastmaster/trapper/fire hybrid R/E is a brilliant build and won't hear any arguments to the contrary, or other such nonsense.
Also, the first level of Shards of Orr has been toned down far, far below what it was on release. It was hard at release. Now, you have to fight like 2 mobs, maybe; everything else has been dispersed so much you can walk past them.

Happened today. We got lucky and got a great monk in RA. Unfortunately our necro we got thought he was frontline material and kept running in. The real peeve? He got mad at us for hanging back initially. Luckily, our monk was really good and we got the 10 wins before a TA team beat us.
